Greetings to my current clients:
I am retiring December 15th, 2021.
There are several options for you to consider depending on your needs or preferences.
- If you feel that you are no longer in need of therapy, this is a good opportunity to remind yourself of the positive changes that you have accomplished during our time together. You may find it helpful to schedule additional session(s) to consolidate your gains and have a sense of closure.
- If you feel that that you are currently stable, though require continued medication management, I can provide pertinent records to your primary care provider to assure continuity of care.
- If you feel that it would be best for you to a see a specialist for follow up, such as a psychiatric nurse practitioner, psychiatrist and/or a therapist, I have tried my best to compile a list of potential community providers that I will share with you. It is ideal to check with your insurance company to determine providers who have availability and accept your insurance. It is very important for you to act as soon as possible to accomplish this so that you can continue to receive needed services.
Please schedule an appointment to discuss what option would work best for you and which potential providers may be the best match for your needs. I want to do what I can to assure that your new provider has the pertinent information to best take care of you. You will find a release of information form that you need to fill out with your new provider’s information. Please let me know if you have any difficulty with this transition.
I also want to assure you that I respect your confidentiality and I will securely store your private records after the close of my practice.
